The first step to getting a new vaccination card is to contact your vaccine provider, according to public health officials. The Ohio Department of Health contacting your local health department if you can鈥檛 contact the vaccination site.
Summit County Public Health has a to request a new card, but the agency can only track down vaccinations that happened in Ohio, said spokesperson Marlene Martin.
If you were vaccinated in another state, you have to contact a health department there, Martin said.
The Ohio Department of Health (ODH) recommends taking a picture of the card on your phone in case you lose the physical copy. ODH also advises against laminating the card, in case booster shots are needed in the future.
